Welcome to the Innovation Institute WIN forms archive. Here you'll find our registration and disclosure forms for both our innvention evaluation and our product assessment services. While we eventually hope to put the forms up in a number of different word processor formats, we currently support Microsoft Word (.doc), Rich-text Format (.rtf), Adobe Acrobat reader (.pdf), and web-viewable versions. If you are unable to access or print any of these versions, please see below for e-mail or postal addresses where you can request forms from.

Please do not submit any materials on your invention/idea or product before obtaining, reading, and filling out a Registration and Disclosure form. Also, if you haven't yet checked them out, you'll want to visit our About I² and WIN page and our Invention Evaluation or Product Assessment FAQ (frequently asked questions) pages; they contain important information on I², WIN, and the PIES-XII Evaluation System and the PAS-III Product Assessment System (if you request forms by mail you get a subset of this information).

If you are unable to download or print any of the online versions of our forms, you can e-mail us at forms@innovation-institute.com to request that they be mailed to you (be sure to specify either an invention evaluation or a product assessment Registration & Disclosure form).
